Below are some commonly-used forms that Human Resources offers online for your convenience. Some of the forms are such that they can be filled out and submitted electronically (if this is the case, “electronic” is indicated next to the selection). All of the forms can be viewed using the “PDF” option, which utilizes the Adobe Acrobat Reader to bring the document up on your screen in an easily readable and printable format (you may then fax or mail the form to Human Resources).
